Job Description

Soleo Health is seeking a Nurse Ambassador to work Remotely. The nurse ambassador will skillfully guide patients, caregivers, and key providers through Soleo’s home infusion process. In this hybrid role, the nurse ambassador is expected to use a multi-disciplinary approach to gather and organize pertinent information in order to provide the patient with a streamlined home infusion experience.

Job Responsibilities

  • Effectively communicate prescribed therapy in the context of a home infusion/specialty experience
  • Introduce patients to our patient advocacy resources
  • Identify nursing needs and coordinate with internal or inter-agency nursing resources
  • Plan and coordinate the scheduling activities and associated tasks for the nursing department
  • Demonstrate accountability and responsibility for own work
  • Provide daily and/or weekly updates to management
  • Collaborate with prescribers to facilitate nursing needs
  • Provides exceptional customer service to external and internal customers, resolving any customer requests in a timely and accurate manner
  • Ensures the appropriate notification of patients in regards to their nursing needs
  • Promote, foster and maintain optimal relationships across departmental and company lines to influence positive results
  • Meet current documentation standards and policies with in electronic medical record
  • Supports client satisfaction at a level that ensures account retention

Requirements / Qualifications

  • LPN/LVN/MA preferred but not required
  • Minimum three (3) years of clinical experience and/or home health field or management experience required
  • Knowledge of hospitals, skilled nursing/rehab facilities including case management and discharge planning service needs
  • Good understanding of pharmacy procedures and practices
  • Intermediate level skill in Microsoft Excel (for example: using SUM function, setting borders, setting column width, inserting charts, using text wrap, sorting, setting headers and footers and/or print scaling)
  • Intermediate level skill in Microsoft Word (for example: inserting headers, page breaks, page numbers and tables and/or adjusting table columns)
  • Intermediate level skill in Microsoft PowerPoint (for example: applying a theme, formatting character spacing, inserting a picture, changing slide layout and theme colors, adding transitions, customizing slide numbers, changing chart style and/or formatting font)
  • Highly effective in working objectively with a diverse group of people and must demonstrate communication, organizational, administrative and office managerial skills
